Do we treat your condition?
We treat any musculoskeletal disorder (sprains, strains, tears, pre/post-surgical) and some neurological diagnoses. If you're looking for ongoing pediatric therapy, please email us for more information.
Do we accept worker's compensation insurance?
Yes: we communicate and bill 3rd party providers (Sedgwick, EMC, Broadspire, Corvel, etc) directly. You can either email us your paperwork or fax it to 651-327-2883. If you're not sure where to start, leave us a voicemail and we'll give you call.
What is Rapid Response Care?
Rapid Response Care (RRC) is a way for employees injured at the workplace to receive a prompt physical therapy evaluation and get started on a rehab plan right away. Some cases can remain classified as *OSHA first aid care (non-recordable) if intervention is limited. Some cases will require formal treatment, referral for imaging or further evaluation and become a work comp case. We utilize conservative care first whenever appropriate.
*To remain classified as OSHA first-aid, your company must have a Rapid Response Care plan already in place with ETHOS. Contact us for a free consult if you're interested.
Insurance Vs. Self-Pay
We provide a time of service discount of approximately 50% off insurance contracted rates. If you do not typically meet your deductible or are out of network, this is a way to save money. You can use an HSA for payment if you have one.
We have in-network agreements with most major insurance companies. We ask that you verify your own coverage by calling the number on the back of your insurance card. You will want to know:
- Have you met your deductible and how much is it?
- Do you have a per-visit co-pay ($ per visit or % per visit?)
- Do you need a referral or pre-authorization?
